Ideas:

The current ProofreadPage extension isn't the same code as what WS has. Not sure if this is the root of the problem, but it's definitely true that some pages aren't identical.
- proofread_index.js
IndexForm.js calls something we haven't defined.

*Script importing doesn't work at all It is working; IndexForm.js is adding text to Index:Blah.djvu.
