Ideas:

IndexForm.js calls something we haven't defined.

*Script importing doesn't work at all It is working; IndexForm.js is adding text to Index:Blah.djvu.

One of the MediaWiki setup pages is not configured properly
I didn't copy correctly from WS
The current ProofreadPage extension isn't the same code as what WS has.
